I created a sample questionnaire (iPhone Users Questionnaire #1)。
After downloading this app, push "answer the questionnaire" button and input the following.

Number ： 20140729
Password ： apple

After you downloaded the sample questionnaire, check any choice and swipe to next question, and finally push "Submit" button. Though the view will back to "Queationnaire List" table, click the questionnaire again. So you can see the toolbar button on the upper right side of the navigation bar. Then you can see the results of this questionnaire and send with E-mail or post to Facebook and twitter. Of course, from the "Create a Questionnaire", anyone can create a questionnaire easily.
